Wholesale Buying Application and Purchasing Policy

All information submitted is confidential and intended for the sole use of Glass Mosaic America. Complete the above forms and return them to Glass Mosaic America along with:

• Your business card
• A copy of your Provincial tax license if you are an Ontario based merchant (if your province does not have a Provincial sales tax, a copy of your Federal or Provincial business registration may be substituted)
• An initial minimum first purchase of $1000.00 to qualify for our wholesaler discount
